BUILDING EXTENSION - PRE-QUALIFICATION

NOTICE FOR PRQ-QUALIFICATON OF CONTRACTORS

Pakistan Institute of Prosthetic & Orthotic Science (PIPOS) Hayatabad, Peshawar invites reputable contractors, registered in category C-3 or above with Pakistan engineering Council, to submit their credentials for pre-qualification of Extension of existing (PIPOS) building.

Interested contractors/construction firms may apply by providing the following information within 15 days of the date of this advertisement:

1. Full name address (with Telephone /Fax. No & E-mail / Address etc.) of firm.
2. Organization structure (company or partnership) with a certificate and particulars of the partners and Directors along with copies of their National Identify Card Certified copies of the articles/memorandum association of partnership.
3. List of the equipment owned by the firm (verifiable).
4. List and cost of projects in  hand indicating name of the client, consultant and the scope of work.
5. Details of similar works last completed along with completion certificates (verifiable)
6. Up to date registration with Pakistan Engineering Council in category C-3 & above.
7. List and Bio data of key personal for different engineering works (verifiable).
8. Financial statements and bank reference (verifiable).
9. National Tax Number and Income Tax clearance certificate for the last year.
10. Affidavit that the applicant's firm is not black listed or disqualified by any Govt./semi Govt. Department.
11. Affidavit to the effect that the firm is not involved in any litigation with any department in the past and present.
12. Affidavit on stamp paper properly attested submitted by the contractor/firm that the statement referred above is correct and nothing has been concealed intentionally.

Note: (PIPOS) reserves the right to pre-qualify or reject any or all the applications without assigning any reason thereof. Tender documents shall be issued to the pre-qualified contractors only. Application received after due date will not be entertained.
